UPDATED FORM POLICY

We have had an influx of form requests. To deal with the increase in forms (often duplicate requests), there will be a $10 fee for all forms not completed at your child’s checkup.

 

Each form will take 2-5 business days to complete. If the form must be completed the same day, there will be a fee of $15. For convenience, forms can be requested and submitted via the patient portal. Forms will be uploaded to the patient portal once complete.

 

Please write the patient’s name and date of birth on each form. Any form submitted blank will be discarded.  We will NOT fill out any forms for medical clearance if the patient has not had a well-care visit within the past year or if the child has never had a well-care visit at our office.

NEW PATIENT ANNOUNCEMENT

As of June 1, 2022 we will no longer be accepting new patients. The doctors current schedule is at capacity, you may contact us early October when we will re-evaluate the doctors schedule. We will continue to welcome expecting [new first time families] newborns.

SICK CHILD VISIT PROTOCOL

We have returned to our curbside protocol for all sick visits (weather permitting). In the event a sick child needs to be seen inside the office, we are asking only one parent accompany the child inside.

FACE MASK

Face masks will be required for all visits, curbside included, for the foreseeable future as we continue to see covid-19, flu, and other highly transmissible illnesses.

COVID-19 ANNOUNCEMENT

 

We wanted to take a moment to update you on what we are doing to help mitigate the risk of COVID-19. We continue to follow the evolving research surrounding COVID-19 and adjust as needed to how we are currently practicing based on recommendations from the CDC (Centers for Disease Control) and WHO (World Health Organization) along with the NJ Department of Health (NJDOH). 

 

For the safety and well-being of our patients and staff, we will NOT be bringing patients into the office with suspected Coronavirus. Our staff will continue to screen ALL phone calls. If your child has symptoms or has had exposure and you are concerned about COVID-19, please call the office for guidance. We are only offering PCR testing at this time. We do NOT have rapid testing available. All tests will be sent out to either LabCorp or Quest based on your insurance. 

 

At this time we are doing our best to separate our wellness appointments from our sick visits.  Depending on your child's symptoms, some office visits may be performed from the comfort of your car. We are currently testing for COVID-19, however, testing is performed at the end of each day and via "drive-up" testing. If your child needs to be tested for COVID-19 or if you have questions regarding testing, please call the office and we will be happy to explain the process for you. 

We are asking that all patients please remain in their cars and call before entering the office.  The staff will notify you when we are ready for your to enter. At this time, both parents may accompany the child to the appointment, however, we ask that siblings and other family members do not attend. If you cannot make arrangements for siblings, please let us know. Everyone, including children 3 years and older, MUST wear a mask while in the office. 

 

Remember, practicing basic hand hygiene, frequently disinfecting commonly touched hard surfaces, and being mindful of other healthy habits is still our best defense at protecting ourselves and loved ones against communicable diseases. Take this opportunity to educate your kiddos on proper handwashing techniques, how to properly cover their coughs and sneezes, and to build the habit of not touching their faces. 

 

We understand that the current COVID-19 outbreak is scary and we want our families to remember that we are here for you. We will continue to do what we can to ease the minds of our families. We will do our best to update our patients and disseminate information as we receive it. Be sure to follow the CDC, WHO, and NJ websites for the most accurate and up-to-date information. New Jersey has also implemented a Coronavirus Hotline in partnership with the Poison Control center. Call 1-800-222-1222 or 1-800-962-1253 (if you’re calling from an out-of-state phone line). They have trained healthcare professionals available to answer your questions 24/7.
